Public Library Board Member Report Form Guide

Instructions to Library Directors:

When filling out the Public Library Board Member Report form, the following clarifications may be helpful.

System Representative:

If the system representative is also a board member, please list them again on the pages provided for trustees.

Home address:

Please provide complete home mailing address for each trustee. Systems use this information to correspond with trustees.
Email addresses are also helpful.

Board Position:

This refers to any office held on the board such as; President, Vice-President, Secretary, Treasurer. It is also helpful to indicate any ex-officio members such as mayor.

Dates of term

Year the current four-year term begins and ends.

Is this the trustee's first term?

This applies to the current term of service. If their first appointment was to fill out an unexpired term for another board member, indicate "first term" as the first full term they serve.

Is this the second term?

This applies to the second full term of consecutive service. If they served in the past and were off the board but have returned to serve a second time, do not indicate their prior service.

Is this trustee filling a term for another board member?

Do not check "first term" if the appointment is to fill out a term for another board member. It is helpful to state whom the new person is replacing.
